Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

We are seeking a Remote Patient Care Coordinator to join a busy US‑based cardiology team. The role focuses on patient communication, scheduling, care coordination, and clinical documentation for a high‑volume caseload of 150+ patients.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, follow‑up appointments, and remote patient monitoring workflows.
• Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA compliance.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical credential such as CNA, MA, RN, or equivalent.
• Minimum 3 years experience in healthcare, telehealth, or medical office settings.
• Strong background in care coordination or management, preferably in a remote or telephonic environment.
• Ability to handle a high‑volume, remote workload with warmth and precision.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
