Clinical Readiness Specialist

About the role

EDGE is seeking a remote Clinical Readiness Specialist to support care teams in preparing patients, providers, and clinics for efficient, high‑quality visits. Reporting to the Director of Clinical Effectiveness, the role focuses on pre‑visit chart preparation, identifying care gaps, and proactive patient outreach.

Key responsibilities

• Review upcoming provider schedules and ensure patient charts are complete, accurate, and visit‑ready, tracking preventive care gaps and HEDIS/UDS measures.
• Identify and flag missing clinical data or documentation for staff review.
• Conduct outbound patient outreach to schedule preventive services, follow‑up appointments, labs, or referrals.
• Document all outreach attempts and outcomes in the electronic health record.
• Serve as a liaison between clinical teams and operational departments to support patient flow and compliance activities (HRSA, Joint Commission, PCMH).
• Maintain tracking tools for care‑gap closure progress and productivity reporting.

Required profile

• High School Diploma, GED, or equivalent.
• Minimum 1 year of experience in a healthcare setting (e.g., medical assistant, care coordinator, patient outreach, population health, preferably in an FQHC).
• Strong written and verbal communication skills; bilingual English/Spanish preferred.

Required skills

• Experience with electronic health records, preferably NextGen.
• Accurate data entry and tracking tool maintenance.
