Caregiver – Residential Youth Support

About the role

Indiana Mentor, part of the Sevita family, is seeking compassionate caregivers to provide direct support to youth in residential and treatment foster‑care settings. You will work closely with individuals to promote stability, health, and personal growth.

Key responsibilities

• Deliver individualized support according to service plans, including socialization, health maintenance, and behavioral development.
• Assist with daily living activities such as personal care, housekeeping, recreation, transportation, and community integration.
• Administer medication or support self‑administration and accurately document all treatments.
• Maintain detailed records of progress, activities, and changes in service plans while ensuring confidentiality.
• Manage budget‑related purchases for individuals, respecting preferences and cost‑effectiveness.
• Build and sustain professional relationships with youth, families, guardians, and case managers.
• Report any suspected abuse, neglect, or incidents in line with internal policies and external regulations.

Required profile

• Ability to perform essential functions such as personal care, transportation, and documentation.
• Strong commitment to confidentiality and respect for the rights of individuals served.
• Willingness to accompany individuals to medical appointments and communicate with healthcare providers.
• Capacity to work in a team environment and follow Sevita Customer Service Standards.
• Readiness to report abuse, neglect, or incidents promptly according to policy.
